Preparing students for the high school years and beyond is a unique time. This period, known as TRANSITION, is a time in which educational planning moves from year to year development to considering the “big picture”. Transition planning usually begins when a student turns 13, but can be addressed earlier given the individual needs of the student. Transition is addressed in stages that are designed to develop into a long term plan. Transition begins by identifying student interests and aptitudes and works toward creating a goal based plan that will help prepare you and your student to reach appropriate post-school goals.
The years of Transition from school to adulthood can be difficult for everyone. The key to successful and appropriate Transition begins with being informed and making educationally appropriate decisions for your child. As your child’s Transition Specialist, I will work with your child’s teachers, and other members of the IEP team to provide you and your child information to help plan for his/her future.
Transition is a coordinated set of activities which is designed to be outcome-oriented and will promote movement from school to post-school activities such as post –secondary education, vocational training, employment, independent living, or community involvement. Transition addresses five major areas and is based upon the individual needs of the students as well as his/her preferences and interests. Depending on the student’s needs the IEP team may focus on some or all of these areas which become the focus of the student’s IEP. The areas of Transition include Instruction(academic or curriculum needs), Post Secondary Education, Employment and/or Adult Living, Community Experiences, Related Services (any services such as vision or mobility, physical therapy or any service received in the school environment which will need to continue upon exiting school), Daily Living(skills adults do everyday).
My goal is to insure that students receive the necessary experiences to insure a successful post secondary transition into society following high school. I am aware each student has a unique pattern of strengths that need to be maximized in order for a successful post secondary adjustment. I look forward to working with you and your child.
